JazzCares Patient Support Services
Jazz Pharmaceuticals, through its JazzCares Patient Support Programs, offers access and reimbursement support and financial assistance options to patients who are prescribed its hematology/oncology drugs (Table).
Patient Support Programs
Patient support is available for Defitelio, Rylaze, Vyxeos, or Zepzelca, and a copay savings program is available for Zepzelca, Vyxeos, and Rylaze. JazzCares provides resources to help patients understand their insurance coverage and find information on sources of financial support.
JazzCares Patient Assistance Program
Patients with no insurance or those who are functionally uninsured or underinsured due to lack of coverage may be able to receive free medication through the JazzCares Patient Assistance Program.
Income and other criteria apply, and documented proof is required. For more information about eligibility and assistance, please contact a JazzCares specialist at 833-533-5299, Monday-Friday, 8 am-8 pm ET.
Application & Enrollment
JazzCares Program online enrollment for Vyxeos, Rylaze, and Zepzelca can be initiated through JazzCares.com or submitted by fax to 855-593-3955. Enrollment for Defitelio can be submitted by fax only.
